By checking, I hearby authorize Browning Enterprises to obtain and review my Motor Vehicle Record (MVR) from any state motor vehicle agency or authorized reporting provider for employment, insurance, driving eligibility, or other laful business purposes. 

I understand that the report my include information regarding my driving history, license status, traffic violations, suspensions, revocations, and accident history.

I certify that the information provided above is true and complete.

I (the applicant) authorize the releasse of this information and releae the company (Browning Enterprises) and reporting agencies from liability arising from the laful use of this authorization. 

This authorization remains valid unless revoked in writing where permitted by law
